Common Access Mistakes
The same access failures recur in building after building - the token ramp, the unusable accessible WC, the colour-only cue, the step to the accessible door, the blocked route. This lesson names each one and shows exactly how to fix it
You will see the same handful of access failures in building after building. Learn to recognise them on sight, and you can design them out before they are ever built.
There is a strange comfort in how repetitive access failures are. Walk any high street, campus or hospital with a trained eye and you will meet the same small cast of mistakes again and again: the ramp so steep no one could push up it, the accessible toilet doubling as a mop store, the sign that means 'exit' only if you can tell red from green, the accessible entrance that itself has a step, the level route with a bin parked across it. These are not exotic or clever failures. They are ordinary, predictable and, once you can name them, almost entirely preventable.
This lesson is a field guide to those recurring mistakes. For each one we do three things: describe how it happens (because almost none of them come from malice - they come from access being an afterthought, a token, or a maintenance blind spot), explain who it actually excludes, and give the concrete fix. Some fixes are yours at the drawing board; some are about how a building is run once it opens, because access is broken as often by daily operation as by design. Learn this cast of characters and you gain the most practical skill in the whole module: the ability to spot a barrier before it is built, and to catch one that already exists in a five-minute walk.
Every mistake here provides an object and forgets the journey. Complete the journey and they vanish.
The token ramp, and the step to the accessible door
Two of the most common failures are really the same failure wearing two faces: access provided as a token - something visibly done so the box can be ticked - rather than as a journey that actually works. Both come from treating access as an object to be seen to provide rather than an outcome to be achieved.
The token ramp is the classic. A level change appears - a step or two at an entrance - and someone adds a ramp to 'make it accessible'. But the ramp is fitted into the space available rather than the length the gradient needs, so it ends up far too steep. A gentle, usable ramp needs a long run for its rise: typical guidance puts the maximum around 1:12, and gentler is better, with level landings to rest on and handrails to both sides (always confirm the binding figures in your jurisdiction). The token ramp ignores all of that - it is short, steep, often handrail-less, sometimes ending at a door that opens outward across it. A person cannot push themselves up it, an assistant risks tipping them, and coming down is worse. It is not access; it is the appearance of access, and it can be genuinely dangerous. The fix is to give the ramp the length it needs, with landings and rails - and, crucially, to recognise when a ramp is the wrong tool altogether. Past a modest rise, a ramp becomes absurdly long; there you need a lift, not a heroic slope. Better still, design the level change out at concept stage so no ramp is needed at all.
The step to the accessible entrance is the token ramp's cousin, and it borders on the absurd once you see it: an entrance signed with the wheelchair symbol that itself has a lip, a threshold upstand, or a step - or an 'accessible' side door that is level but kept locked, reached by an intercom, or opening onto the bins. The message it sends is unmistakable and corrosive: you are an afterthought, admitted round the back, on someone else's terms. It usually happens because the main entrance was designed first, without access in mind, and the accessible way in was added afterwards wherever it would fit. The fix is the principle that governs this whole module: make the main entrance the accessible one, for everyone, from the first sketch. A single level, generous, welcoming front door that everybody uses is not only more dignified than a segregated accessible route - it is simpler, cheaper and better architecture. Where an existing building genuinely cannot have a level main door, the alternative entrance must at least be equally prominent, equally easy, permanently available and never a worse experience.
A ramp too steep to climb is not access - it is the appearance of access. And a step to the accessible door is a contradiction.
The 'accessible' toilet that isn't, and colour-only cues
The next two mistakes are the most common failures inside the building: the accessible WC that cannot actually be used, and information carried by colour alone. Both pass casual inspection and both fail the very people they are meant to serve.
The unusable accessible toilet is heartbreakingly frequent because a room can look right and be wrong in a dozen quiet ways. The most common is the simplest: it becomes a store. Because it is the largest and least-used toilet, staff fill it with the mop bucket, the spare chairs, the cleaning trolley, the Christmas decorations - and the transfer space that makes it usable disappears. But the design failures are just as common. The grab rails are in the wrong place or the wrong type, so there is nothing to pull against where the body actually transfers. The door opens inward, eating the very space a wheelchair needs to get in and close it. The pan is set at the wrong distance from the wall for a side transfer, or handed the wrong way for the layout. The emergency pull cord is tied up out of reach, or cut short, so a person who falls cannot call for help. The taps, lock and flush need a firm grip. Each of these is small; together they turn a compliant-looking room into one a disabled person cannot use alone. The fixes are correspondingly concrete: protect the transfer space as sacred - never let it become storage; get the grab rails right for the transfer; keep the door swing out of the clear space; hang the emergency cord to the floor and train staff never to loop it up; and choose fittings usable with a closed fist. And check the handing: many buildings need both left- and right-hand transfer layouts so a person is not defeated by the one side they cannot use.
Colour-only cues are the classic sensory mistake, and they exclude a large group quietly: the roughly one in twelve men (and fewer women) with colour vision deficiency, plus anyone with low vision, plus everyone in poor light or glare. The failure is using colour as the only carrier of meaning - a red light for 'engaged' and green for 'free' with no other difference, a wayfinding system where the only distinction between routes is their colour, a form where errors are shown just by turning a field red, a plan where 'you are here' is a colour a colour-blind visitor cannot pick out. The information is genuinely invisible to people who cannot make that colour distinction. The fix is simple and absolute and worth memorising: never rely on colour alone. Always pair colour with a second, non-colour cue - a shape, a word, an icon, a pattern, a position, or a strong difference in lightness (luminance contrast), which survives colour blindness in a way hue does not. Colour is a wonderful reinforcement and a poor sole messenger. Use it to strengthen a cue, never to carry one by itself.
The blocked route, and the barriers of operation
The last cluster of mistakes shares a distinctive feature: the building was often designed correctly, and then use broke the access. These are the failures of operation and maintenance, and they matter enormously because a route is only as accessible as it is on the day someone actually needs it - not on the day it was drawn.
The blocked accessible route is the most common of all, and the most maddening because the design was fine. A perfectly good level, wide route is rendered useless by things placed across it after handover: an advertising A-board on the pavement, a bin left out, a delivery pallet, a chained-up bicycle, a car parked across the dropped kerb, a fire door wedged with a box, tables and chairs spreading from a cafe, a mop-and-bucket 'wet floor' obstacle course. For a blind cane user these are also collision hazards; for a wheelchair or pram user they are a full stop. The design fix is to give the route enough generosity and clarity that it is obviously a route and not a place to dump things, and to keep it free of the ledges, recesses and dead corners that invite clutter. But the real fix is operational: the accessible route must be protected as a permanent rule of how the building is run - written into leases and staff training, enforced against A-boards and parking, and checked as routinely as the building is cleaned. Access designed and then abandoned to daily entropy is access lost.
The same is true of the features that only work if they are maintained and available: the platform lift that is out of order (and out of order for weeks because no one is responsible for it); the automatic door switched off to save energy; the hearing loop that was installed, never tested, and has not worked in years; the accessible parking bay used by non-disabled drivers because it is never enforced; the tactile paving worn smooth or paved over at the next resurfacing. Each was provided and each has quietly failed. The lesson here reaches beyond drawing: specify features that are robust and maintainable, assign someone responsibility for keeping them working, and design so that when something does fail there is a fallback - because a single point of access with no alternative is one breakdown away from exclusion.
There is a pattern running through every mistake in this lesson, and naming it is the real takeaway. In each case, access was treated as a thing - a ramp, a room, a symbol, a feature - rather than as a continuous journey completed by a real person. The token ramp, the step to the accessible door, the toilet-turned-store, the colour-only sign, the blocked route: all of them provide an object and forget the person's actual passage from arrival to use to safe exit. Fix the mindset - complete the whole journey, for the real person, and keep it working - and these mistakes become almost impossible to make.
A route is only accessible on the day it is needed - not the day it was drawn. Protect it as a rule, not just a line.
Turning the mistakes into a five-minute test
The value of knowing the recurring mistakes is that they compress into a fast, practical test you can run on almost any building in a few minutes - the beginnings of the access-audit skill covered in Module 9, distilled to the failures you will actually meet. Once these are in your eye, you cannot switch them off, and that is exactly what you want.
Run the journey, in order, asking the mistake-questions as you go:
ARRIVAL Is there accessible parking near the entrance, and enforced?
Does a level route lead from it to the door, with no kerb?
ENTRANCE Is the MAIN entrance level - or is access a step, a lip,
a locked side door, a token ramp too steep to use?
MOVING Is the route clear RIGHT NOW - no bins, boards, clutter?
Can a wheelchair turn where it needs to; can two pass?
USING Is the accessible WC clear (not a store), rails right,
cord to the floor, door swing clear, fittings fist-usable?
SEEING Is any critical cue carried by colour ALONE?
Is there real light/dark contrast on edges and signs?
WORKING Do the lift, auto-door, loop and paving actually work today?
LEAVING Could this person get OUT in an emergency, not just in?The last line matters more than any other and is the one most often forgotten, so it is worth dwelling on: a building that a disabled person can enter but not safely leave in a fire has failed at the most fundamental level. Getting out is covered properly in Module 8, but include it in every quick test from now on - access that stops at the front door is not access.
Use this test on your own designs before they are built, on precedents you visit, and on your own daily journeys until it runs by itself. When you can walk into a building and silently clock each of these in a couple of minutes - level main entrance, yes; route clear, no, there's an A-board; accessible WC, can't tell, it's locked; contrast, weak; egress, no refuge - you have the single most useful practical skill this module can give you. The mistakes are common precisely because so few people can see them. Once you can, you will design them out by reflex, catch them in review, and quietly make every building you touch work for more people. That trained eye, turned on the recurring failures, is what separates a designer who means well from one who reliably delivers access.

Ramp gradient (~1:12 typical maximum)

The token ramp ignores this. Gentler is better; past a modest rise, use a lift. Verify the binding gradient, landing and rail rules in your local code.
Accessible WC transfer space + grab rails

Must be kept clear (never a store), rails correct for the transfer, cord to the floor, both handings considered. Confirm dimensions and handing against current guidance.
Never colour alone (luminance contrast)
Critical information must not rely on hue as its only cue
Pair colour with shape, text, icon or light/dark contrast, which survives colour blindness. A core rule from the sensory-access module.
The unobstructed accessible route
The route must be clear and working in use, not just on the drawing
Blocked by bins, boards, parking or broken equipment - an operational failure. Protect it as a rule of how the building is run. Verify against RPwD Harmonised Guidelines.

Most of these mistakes are yours to design out at concept stage - the step to the door, the impossible ramp, the stranded WC, the route with no generosity. Set the main entrance level for everyone, size the accessible route so it reads as a route and resists clutter, place the lift where it truly serves every floor, and never let 'accessible' mean a locked side door by the bins. And design the egress: a person who can get in must be able to get out. Get the big moves right and the token fixes never become necessary.
The unusable accessible WC and the colour-only cue are squarely in your scope, and both are avoidable. Protect the transfer space, get the grab rails and handing right, hang the emergency cord to the floor, keep the door swing clear, and specify locks, taps and flushes usable with a closed fist. Never let meaning ride on colour alone - back it with shape, text, icon and real luminance contrast. And specify fittings and finishes that stay usable and are easy to keep clear, so daily operation does not quietly undo your work.
